Architectural Control Committee (ACC) Candidate Form

The committee that reviews applications for alterations to your property can be called many names per your Governing Documents. We are using the term ACC to represent all of the various names. ACC, ARC, AC, etc. Please fill out the below and submit this information to DAMC by the date requested by your Association. Information submitted late will not be considered. None of the following will be used to discriminate in any manner.
